2022-23 High School Community Service Scholarship Application

Miramonte, Campolindo, and Acalanes High Schools
are eligible for these scholarships

Please finish the application in one session before submitting.

  • Note: you will need to start and finish typing the online application in one session, so consider typing it in another document and using “copy and paste” to create your submittal.


  • List the extracurricular activities in which you have participated. State the number of hours per week, number of weeks per year, total number of hours per year. and the number of years committed to the activity. Briefly identify the goal of the program. and include a description of your role in the program.


  • List the volunteer activities in which you have participated. State the number of hours per week, number of weeks per year, the total number of hours per year, and the number of years committed to the activity. Briefly identify the goal of the program and include a description of your role in the program.
  • Please list your two references: (1) a school advisor, teacher, or counselor and (2) your community service representative.


  • Please write an essay of up to 500 words explaining how you grew as a person through community service projects and high school extracurricular activities. The essay is to answer the following questions:
    • In thinking about your service to community and school, which activity gave you the greatest sense (or feeling) of success and accomplishment and why?
    • What did you feel your community service taught you about yourself and others?
    • What are your plans for college?
    • How might you continue your community service experience during your college years?

Your next steps in the application process: 

  • Contact the two references and ask for letters of recommendation – the school representative who knows your work at school and the community service representative who has experience working with you or supervising you during your service or project.
  • Obtain a copy of your transcript from the school.
  • Assemble the letters of recommendation and transcript.
  • Place all the material in an envelope, addressed to:

Deliver the envelope to your counselor at the College and Career Center
by Tuesday, February 15, 2023, by the close of the school day.
